AB0297 MORTALITY AND DISEASE RELATED COMORBIDITIES IN SYSTEMIC LUPUS ERYTHEMATOSUS: DATA FROM AN EGYPTIAN COHORT WITH SYSTEMIC LUPUS ERYTHEMATOSUS- PHASE I

Background: Systemic lupus erythematosus (SLE) is a complex autoimmune disorder with considerably high mortality. Objectives: To investigate the survival rates 5, 10, 15 and 20 years impact of disease related morbidity mortality among Egyptians patients afflicted by SLE. Methods: This single center observational study performed in one leading medical school governmental hospital for teaching training north African region middle east sectors Kasr Alainy School Medicine- Cairo University. Medical records adult SLE ≥ 16 (classified according to ACR 1997 classification criteria set forth Hochberg, 1997) who received longitudinal clinical care during time period from 1999 2019 were included. Data analysis: causes mortality, damage score determined diagnosis last contact or date death. Results: Records two hundred included, 91.1% females 8.9% males (ratio 10:1). The mean age at 26.71 ± 7.93 follow up between mean: 6.6 4.58 years, 34.15% had least organ systems SLICC/ACR-DI first 6 months. Considering an outcome label dead alive end period, results showed total 52 mortalities, 88.5% females, death onset was 30.9±8.8 years. Results identified following causalities studied order frequency: Septic shock disseminated intravascular coagulation 11.5 %, acute respiratory distress syndrome ARDS congestive heart failure 9.6%, thrombotic microangiopathy 5.7%, cerebritis, renal intracranial hemorrhage hypertensive encephalopathy 5.7%. Alveolar hemorrhage, infection, intraoperative deaths each contributed 3.8%. Hypovolemic shock, liver failure, brain edema, thrombocytopenic purpura, stage kidney disease, pulmonary syndrome, suicide hydrocephalus fatalities 1.9%. cause unclearly 26.9%. Kaplan Meier curve cohort overall cumulative probability after 82.9%, 68.8%,51.4% 20.4%, respectively. Multivariate regression analysis revealed psychosis, chronic independent predictors (HR= 4.3 times, 3.58 times 3 respectively, p < 0.001), while use hydroxychloroquine AZA protective effect.
